We are thrilled that you’ve decided to become a new patient of Dentistry by Cure Clinics in Fulwood, Preston.

At Dentistry by Cure Clinics, our philosophy is simple. It’s getting to know you, building lifelong relationships and being there when you need us. Our team is dedicated to providing you with calm, comforting oral healthcare utilising the latest innovations and technology dentistry has to offer.

This membership is our commitment to you, and your families when it comes to providing first-class oral healthcare. Our membership, at its heart, is based on being preventative, making oral health a priority, personalised to you, for you and participatory; empowering you to make informed decisions about your oral health.

This document will help you to understand what you can expect as a patient: what’s included in the membership, and what treatments or services are not covered under this membership agreement. We will outline the terms and conditions which constitute an agreement between you ‘the patient’, and your dental provider, ‘Dentistry by Cure Clinics.’

If any aspect of this agreement doesn’t make sense, please ask a member of the dental team who will happily help you. Completion of your online membership form constitutes a record of this agreement being approved, and the anniversary date of commencement for your membership benefits. Our dental membership is based on a subscription. This means you can get some of the great benefits of your dental membership right away when you join.

As part of your membership, you’ll also be invited for an in-depth, new patient appointment which will give us an opportunity to make a great first impression and set the tone moving forward for developing a positive and meaningful relationship.

It’s important to be clear, you do not need perfect teeth to join our membership- we’re here to help you on the way to improving your oral health.

So, how does our Adult membership work?

  • Your adult membership includes your essential appointments at the Practice
  • Your new patient appointment
  • Twice-yearly healthy smile checks with our dental team
  • Twice-yearly hygiene-therapy maintenance & personalised oral health care advice
  • Access to dental care 7 days a week with early and late appointments
  • Free dental prescriptions for appropriate pain relief and antibiotics
  • Access to our refer a friend scheme. Use of a personalised code by family and friends, saving both you and them money on your membership.
  • Ability to redeem an urgent appointment for toothache or trauma if needed (limited to one/year) and access to emergency care over 7 day’s week.
  • Exclusive access to our children’s membership (see below)

Our children’s membership-

  • Is only accessible with a registered adult member
  • Under the age of 16, this is a complimentary membership
  • This includes twice-yearly children’s oral health checks and personalised preventative care.
  • Access to emergency care for pain or dental trauma 7 days a week. 

So what’s not included? 

As you can see from above, you’re getting a lot of value in your essential appointments, which are fundamental to achieving and maintaining your oral health. However, extra care might be needed. Investing in your oral health will absolutely pay dividends when it comes to your ‘Quality of Life’- being able to smile confidently, chew efficiently and sleep peacefully.

  • Everyday dentistry- such as routine fillings, advanced periodontal (gum) treatment or simple extractions
  • Root canal treatment (routine and complex)
  • Additional, or multiple emergency appointments
  • Crown and bridge work
  • Dental appliances- dentures, mouthguards, or splints
  • Orthodontic treatment and orthodontic retention
  • Cosmetic dentistry- from stain removal to tooth whitening and veneers
  • Sedation care- to ease those worries and anxieties in treatment
  • Dental implants, oral surgery, and complex dental care
  • Facial Aesthetic treatments and Skin care

Paying your membership fees

Our adult membership is based on an annual cost of £330.00

Option 1 - Pay monthly at £27.50 per month as a reoccurring payment

Option 2 - Pay upfront annually at £300.00 and save £30.00

To maintain our high standards of customer experience and clinical care, we review our pricing structure often. If we deem it necessary to increase the cost of your membership subscription, we will notify you, providing at least one month’s notice informing you of the new monthly payment to be collected. Payments will be setup as a reoccurring card payment, facilitated by Stripe, our chosen financial partner.

Your monthly payment

Your reoccurring payment may commence more than 4 weeks before your 1st appointment at the practice, subject to appointment availability and when you complete your membership form(s.)

Your commitment to Dentistry by Cure Clinics

We would be very grateful if you are unable to attend your Treatment appointment to cancel with at least 48 hours’ notice. With all membership related bookings, we require at least 24 hours' notice for re-schedule or cancellations.

We accept, and understand mistakes happen, appointments can be forgotten amid juggling everything else life throws at you. We will communicate all appointment confirmations and reminders via email and SMS. In addition, we may write to you if you lapse your recall interval for your oral health reviews.

With all short-notice cancellations and failed appointments, you will incur financial penalties or be asked to relinquish your membership with Dentistry by Cure Clinics if this is reoccurring issue.

Please let the dental team know if you have any changes to your oral or general health, as we may need to change the way we care for you. On a regular basis, we will ask you to update your medical history. If you fail to inform us of changes, this could result in unnecessary complications managing your oral health.

Renewing your membership

For annual-paying members, there will be an automated renewal payment taken with payment details via Stripe on the anniversary date as initial subscription each year. You will receive communications in advance. For monthly members, your payment will be taken on the same date each month on a rolling basis using payment details via Stripe.

Your Membership

We understand that circumstances change, and you may need to cancel your membership with Dentistry by Cure Clinics. To arrange cancellation of your membership, we request notice of one full calendar month, in writing, so we can cancel future payments on your behalf.

In some circumstances, you may need to discuss this further with your bank. Changes to your membership can also be made in Your

Membership section on our website.

 In the unlikely event you wish to cancel your membership during an active course of treatment, your treatment costs will revert to guest status, and the outstanding balance will need payment prior to completion of your treatment. Failure to pay your account could result in delays or cancellation of treatment.

Failed membership payments

If multiple reoccurring membership payments fail, your membership will be cancelled. Please note this will affect all patients connected to your subscription, such as children’s memberships. All upcoming appointments will be cancelled, and you will only be able to return to the practice on a guest basis.

Variation of conditions

From time to time, we may need to vary the conditions of this agreement to reflect changes in law or regulations. In these situations, you will receive correspondence or notification of any changes made. Clinical Consultations

A Clinical consultation is an appointment to determine if you are suitable to commence dental treatment. It could be Stabilisation care, Orthodontic treatment (Invisalign), Dental implants, or sedation treatment.

Discovery appointments with a treatment co-ordinator are free of charge. No treatment is carried out during these appointments, except for gathering information, such as photos or a digital scan of your mouth. A consultation fee is a charge payable on booking a consultation with a clinician for the time to discuss your options and, create a treatment proposal following this appointment. This fee is non-refundable.

Failed Appointments & Late cancellations:

Membership appointments:

If you fail to attend or cancel with less than 24hours notice, this appointment will not be re-bookable under the benefits of your membership. Your next membership appointment will be scheduled in line with your recall interval. If you wish to rebook your cancelled/missed appointment, please note this will be a chargeable booking outside the scope of your membership.

Treatment appointment:

If you fail to attend or cancel with less than 48hours notice, in the first instance, you will be charged 50% of the planned appointment fee. Payment will need to be made to rebook this appointment. In the event multiple appointments are missed or short-noticed cancelled, you will be charged 100% of the planned appointment fee and will need to pay in full to rebook the appointment.

Booking of Treatment appointment

To book any treatment appointments or to commence treatment plans, we kindly request a minimum 50% deposit of the planned appointment fee or in full, depending on the nature of planned treatment.

Refusal of treatment

We reserve the right to refuse treatment where our clinicians deem it inappropriate for any reason, and the clinician’s decision is final in this regard.

Fair Pricing Policy

Here at Dentistry by Cure Clinics, fair pricing is an important part of our ethos. We want the fees that we charge you as a patient to be great value for you but to also reflect a fair price for our skills and experience -without compromising the quality of materials, our duty to the environment, or the respect we have for our staff and suppliers. Below, we have set out what we will do to try to make this happen.

Making you aware of the fees

We want you to be fully aware of the treatment we are proposing, the reason why we are proposing it and of the fees that you will pay before treatment starts. We also want you to be aware of when your fees will be due. In most cases, this will be at the end of each appointment.

To do this, we will:

  • Display a fee guide on the web site, and make sure it is available on request.
  • Discuss with you the treatment we think is necessary and the reasons why and provide information leaflets for further information about your treatment.
  • Provide a treatment plan, which includes estimated costs for each patient before treatment starts.
  • Provide a new treatment plan with updated fees if the treatment needs to change for any reason.
  • We review our pricing structure periodically and do so on a discretionary basis.

Why do we do this?

We do this to preserve and maintain the standard of our service to our patients.

How can I save money on my dental treatment?

  • Sign up to our membership to save money on planned treatment.
  • For treatment plans over £1,000, you can access finance plans to spread the cost of treatment over a prolonged period. Terms and conditions apply. Treatment can only commence after the 14 day cooling-off period.
  • Payment in full, at the commencement of the treatment plan to receive a 5% administrative discount on the total cost of planned treatment.
  • All our treatments are aimed at getting you dentally fit and making your mouth as low-maintenance as possible, to help significantly reduce your treatment needs in the future.

Complaints

If you have a complaint or concern about the service you have received from the dental team or any of the staff working at this practice, please let us know. We operate a practice complaints procedure for dealing with complaints. Our complaints system meets best practice.

How to complain

We hope that most problems can be resolved easily and quickly, often at the time they arise and with the person concerned. If your problem cannot be sorted out this way and you wish to make a complaint, we will respond to your complaint within three working days, enabling us to establish what happened more easily. If it is not possible to do that, please let us have details of your complaint within 12 months of the incident that caused the problem.

Complaints should be addressed to Dr. Christopher Ball. You may ask for an appointment in order to discuss your concerns. We will explain the complaints procedure to you and will ensure that your concerns are dealt with promptly. It will be a great help if you are as specific as possible about your complaint.

 

What shall we do:

We shall acknowledge your complaint within 3 working days and aim to investigate your complaint within 10 working days of the date when you raised it with us. We shall then be in a position to offer you an explanation, or a meeting with the people involved. When we investigate your complaint, we shall aim to:

Find out what happened and what went wrong

Make it possible for you to discuss the problem with those concerned, if you would like this

Make sure you receive an apology where this is appropriate

Identify what we can do to make sure the problem doesn’t happen again

Should a patient make a complaint or claim, we may need to provide information about the patient, and treatment they have received, to insurers, indemnifiers or legal advisers.

 

Complaining on behalf of someone else

Please note that we keep strictly to the rules of clinical confidentiality. If you are complaining on behalf of someone else, we must know that you have their permission to do so. A note signed by the person concerned will be needed, unless they are incapable (because of physical and mental illness) of providing this.

 

Complaining to Dental Complaints Service

We hope that, if you have a problem, you will make use of our practice complaints procedure. We believe this will provide the best chance of putting right whatever has gone wrong and an opportunity to improve our practice. However, this does not affect your right to complain to an independent body, if you feel you cannot raise your complaint with us or you are dissatisfied with the result of our investigation.
